By completing the online booking you agree to these terms and conditions
I understand that the eyelash extension procedure requires synthetic eyelashes to be glued to my own natural eyelashes.
I understand that it is my responsibility to keep my eyes closed and remain still during the entire procedure until my eyelash technician addresses me to open my eyes.
I understand that I am required to follow the eyelash extension care sheet or aftercare which was also verbally explained in order to maintain the life of my eyelash extensions.
I understand failure to follow aftercare sheet and guidelines which were also explained verbally by my lash artist may result in poor retention, bacterial infections as well as the refusal of service by Lashes4u.
I agree to disclose any allergies and/or allergic reaction that I may have/had to latex, surgical tapes, CYANOACRYLATE (eyelash extension adhesive), etc.
I agree to disclose any eye disorders that I may have.
I agree that by reading and agreeing to this consent form, I release Lashes4u for any claims or damages of any nature.
I agree that I have read and understood this consent form.***
I am of sound mind and fully capable of executing this waiver for myself.
I give Lashes4u permission to show me before and after photos of eyelash extensions to other potential clients as well as post, any pictures to the company website and social media accounts.
I have read and completed the eyelash extension intake form and consent form in its entirety, and have answered everything to the best of my ability. ***
I have been informed of potentially harmful or negative side effects that may be caused by the application and/or removal or eyelash extensions.
I confirm and agree that I wish to engage in the services of Lashes4u to apply for my eyelash extensions.
STANDARD TERMS AND CONDITIONS
Deposits:
In order to secure a full set appointment existing client or not, a deposit is required. Please note, a $20 or $40 deposit (depending on the service) is required to secure your spot. This amount is non-refundable and is added to your full set amount on the day of your appointment.
Foreign Refills:
Performing fills on another technician work who is not employed at Lashes4u: We do not fill on other techs work. Quality of work is not guaranteed and it is more work for your Lashes4u artists to attempt to fill on a set done by (more often than not) an unqualified, inexperienced lash tech. Please book in for a removal if you are coming in from another lash salon.
Booking Refills:
Lashes4u is typically booked one month in advance. We strongly recommend prebooking your appointments in advance to avoid disappointment. Last-minute availability is very rare.
Change of Service:
If you require any change or additional services, these must be booked prior to the appointment to ensure the appropriate time is reserved for any appointments. Your appointment time and/or day can be amended through the confirmation email that you receive once you have completed the online booking. Failure to cancel a double booking or an appointment that is not needed will be charged a $50 cancelation fee.
Product purchases are non-refundable.
NO SHOW POLICY FOR APPOINTMENTS:
Failure to not show up for an appointment without providing 24 hours notice of cancellation, will be charged 100% of the service.
CANCELLATION, LAST MINUTE CHANGE TERMS & CONDITIONS:
If the appointment is canceled LESS than 24 hours before the time of the appointment, a $50 fee will be charged to the customer which will need to be paid prior to rebooking.
You will receive 3 emails for your appointment. First email will be sent immediately after booking your appointment. The second email will be the 48hr email that allows you to change your appointment if necessary and a 24hr reminder is sent through text message. You may go online to our booking system and change or cancel your appointment at your convenience. Should you choose to keep your appointment after the second email is sent, you have 24 hours to cancel or change to avoid any fees.
You will not be rebooked until all fees are paid in full.
Early Arrival:
We love for our clients to be here either on time or 5-10 minutes earlier. Please do not arrive 15-40 minutes earlier expecting to be taken in before your scheduled appointment time. That time that we are done early may be the only time that we can reply to emails, use the ladies' room, eat, etc.
Late Arrival:
Lashes4u needs every second of every minute for a booking. If you are late that time will be deducted from your lash session. We are booked back to back with very limited time in between. So, please prepare yourself for traffic, weather conditions, construction etc. Punctuality is an absolute must!
Data Input:
Please make sure that when you are booking your appointment through our website that you are putting in the correct information. First and last name, number, and email. Confirmation emails and texts are sent out and if you miss these reminders it will result in your missing your appointment.
SERVICE RETURN POLICY:
We do not give refunds on services rendered. Due to the nature of the service, please notify Lashes4u within 3 days of service date with any problem and/or concerns regarding your service. We will gladly correct the problem at our earliest convenience, as refunds will not be given.
